Phase I Garage - Student Application Form

This application is required to be completed in full to be considered for access into the Phase I Garage. Submittal of completed application does not automatically qualify you for acceptance. Decision of access will be made by the Garage Committee in accordance with the Access Policy. Applications are reviewed on a monthly basis during the spring semester. Deadlines to apply are: February 15, March 15, and April 15.

Do you have a professor or mentor who will support your efforts while you are in the Garage? If so, who?*
List your involvement with the Entrepreneurship Initiative or any of its affiliated programs (i.e. entrepreneurship courses, UEN, EEP, etc.). If the project is for a particular class, list the class.*
Do you have any background related to entrepreneurship? If so, please describe.*
Describe any personal examples of success or a “can-do” attitude.*
Is this a team project? If so, who else is on your team? How multi-disciplinary is your team? *
Submit a proposal. Note that application will be rejected if the proposal is not at least 250 words. The proposal should answer the following: Why do you want access to the Garage? If it is a business plan, include an executive summary. If it is a project idea, summarize the project. If accepted, your proposal will be made available to other members of the Garage.*
